May I suggest that the forms be posted with just the description of work done and aircraft type, leaving off identification, etc. This would provide sample wording, and if someone wants to get more info they can contact the person who had the word done. This minimizes the risks you pointed out, while still providing a starting point for people who wish to duplicate the work.
