Look at it again. It's two short paragraphs, and in the middle of the second one it tells you how to get it. The sentence starts "If you would like a copy of the document (it's in Microsoft Word 97 format), please send an e-mail to .....".
I'm going to be out of the country and essentially incommunicado most of this week, but I'll respond when I get back. Ernie
